DoxyPEP (post-exposure prophylaxis) is a new prevention strategy that involves taking a single dose of antibiotics within 72 hours of condomless sex to reduce the risk of acquiring bacterial sexually transmitted infections. Clinical trials have shown that DoxyPEP reduces chlamydia acquisition by approximately 88 per cent, syphilis by 87 per cent, and gonorrhoea by approximately 55 per cent in men who have sex with men and transgender women.
The dosing protocol is straightforward. Take the prescribed dose within 24 to 72 hours after condomless sex. Do not exceed the prescribed dose in any 24-hour period, regardless of the number of sexual encounters. If you have sex on multiple consecutive days, take one dose daily. The medication is an inexpensive and widely available antibiotic with a well-established safety profile.
DoxyPEP is primarily recommended for populations with high rates of bacterial STIs, particularly men who have sex with men and transgender women. It is often used alongside PrEP as part of a comprehensive sexual health prevention strategy. Your Specialist GP will assess whether DoxyPEP is appropriate for your individual risk profile and sexual practices during your telehealth consultation.
An important consideration with DoxyPEP is the potential contribution to antibiotic resistance. This is an active area of research and surveillance. DoxyPEP does not replace regular STI screening. Quarterly screening remains essential for all people using DoxyPEP, as it does not provide complete protection — particularly against gonorrhoea — and breakthrough infections can occur. Prevention is most effective when combined with regular screening — Clinic365 recommends a full STI screen at least every three months for individuals at higher risk.
All prevention through Clinic365 is PBS-eligible where applicable, keeping out-of-pocket costs low. Your GP will explain the dosing regimen, monitoring requirements, potential side effects, and what to do if you miss a dose or experience an exposure. Ongoing management is designed to be as simple as possible — with quarterly refills available for $39 online and pathology referrals sent by SMS. If your circumstances change or you have questions between appointments, telehealth consultations are available for $59.
